Digital Enrollment Forms
Our enrollment process is completed through our online class management system brightwheel. Forms can be accessed through the provided link or can be sent to parents via email, to be digitally signed and submitted for enrollment.
The following 7 digital forms are required for enrollment:
(ALL 7 MUST BE SUBMITTED BEFORE ENROLLMENT CAN BE STARTED!)

Basic brightwheel Enrollment Information: Please make sure all required fields are filled out. Phone number and email address are required and make it much easier for us to get back to you!

Schedule Selection: This form allows you to fill in your requested schedule. Current enrollment and availability is how determine if we can meet your scheduling needs.

AZ DHS Emergency Contact Form: Downloaded this form as a PDF, fill it out via computer or mobile device, save it with a new name and upload new saved file for digital submission. The form must be filled out entirely (both pages). Make sure to include any and all important information you feel we should know about your child. You MUST list 2 additional contacts other than legal guardians AND a healthcare provider.

Parent Handbook: Can be downloaded and saved to your computer or device and/or printed for later reference. This agreement requires a digital signature and submission.

Gym Activities Waiver: Gives permission for your child to participate in gymnastics as part of our preschool curriculum.

Little Dreamers Preschool Photo Release: This digital form outlines how and where your child's photo may or may not be used per your digital consent.

Immunization records: This allows form allows you to digitally upload your child's immunization records directly to us via brightwheel.

$50 Registration Fee: Due at time of enrollment, can paid by either cash, check, money order or via your newly created brightwheel account!.
